Hearing, Vision, BMI Screenings
Hearing/vision screens are completed on students in Pre-K, Kindergarten, 1st, 3rd, 5th, 9th, and 11th grades. New students to our district will also receive a screening. If your student is a part of this testing, it is possible that referral paperwork will be sent home regarding professional vision and hearing testing.  Your child should be seen by a dentist two times a year and should also have yearly vision checkups. Please feel free to call me anytime with any concerns.

Immunizations
Students need vaccinations prior to Pre-K, Kindergarten, 7th, 12th  grade entry.If your child needs a vaccine and your local pediatrician is booked, visit an urgent care or the Pike Co. Health District. Your child must have all immunizations required in Ohio within 15 school days of starting or he/she will be excluded from school. Exemption forms must be filed each year and submitted to the school nurse during the same timeframe.

Medication
If your child takes a medication prescribed or OTC at school, you must complete (along with the prescribing doctor) a form for administration, per school policy. It is also the parent/guardians responsibility to refill your child’s prescription and transport it to school. DO NOT SEND MEDICATION WITH STUDENTS OR SIBLINGS ON SCHOOL BUS OR TO SCHOOL WITH THEM. Children who self- carry any medication including inhalers must also have on file an order from their doctor as well as a self-carry formand back up medications must be kept in the nurses office yearly.

Illness Guidelines
If your child is ill in the morning (fever, vomiting, and diarrhea) please do not send them to school. If they do not feel well, (but have none of the below problems) please have a plan and inform the child where you can be located. Our goal is to keep children in school, butyou will be notified if they have a fever of 100 degrees or higher, vomiting, diarrhea, undiagnosed/suspicious rash or head lice. It will be expected that your child be picked up in a reasonable amount of time. If your child is sent home with a fever or had one at home, they should not return to school until they are fever free for 24 hours. If they have been given antibiotics, please do not send them to school until they have had that medicine for a full 24 hours.

Head Lice
If your child has head lice, they will be sent home. If only nits (eggs) are present, they can stay here at school. If your child is sent home they MUST be treated before entry to school the next day and must be checked by the school nurse/nurse’s aide for reentry. The bus will not pick up your child and you must bring them before 9am or your child will be sent home. 

Emergency Medical Forms
It is very important that emergency forms have current information, especially PHONE numbers and health problems, so that in the event of an emergency, there is a way to find your child's medical history, allergies and allergic reactions. Please notify the school whenever changes have occurred.

Please check with the classroom teacher when planning activities or snacks as some student have food allergies.
